2018 brings us the ongoing recognition of the importance of population health with an increased emphasis on patient-centered care. What better way to begin that journey than to consider goals of aging and how to elicit and understand the information obtained. We will have an interactive session with an experienced geriatrician/palliative care physician regarding the intricate process of ascertaining an individual’s goals. This will help guide us through the day as we consider other topics of medical concern, as well as social and psychologic dilemmas.
A novel approach to classifying older adult problems, and thus consideration for goals, is through attention to the four M's: medication, mobility, mentation, and what matters.
Throughout the day there will be opportunities to hear about varying geriatric topics that can often be categorized into these themes.
